Please gather the correct documents before you begin, scan to PDF and save on your computer prior to beginning the approximately 30-minute online session.

Pro Tip
Scanning your documents into PDF format using your phone before you start the online session will save you about 15 minutes.
PDF images are preferred to picture images.

* Choosing the wrong option may give you an error when entering your student's DOB *

Provide legal contact information for parents/guardians and emergency pickups.
